In the past year, have you:
1. Had times when you drank more or for longer than you meant to?
2. Tried (or wanted to try) multiple times to cut back or stop drinking, but you couldn’t?
3. Spent a lot of time drinking or getting over the effects of alcohol?
4. Felt a strong need, urge, or craving to drink?
5. Started to drink alone when you only used to drink socially?
6. Drink most days when use used to only drink occasionally?
7. Found that drinking or its effects kept you from doing work, going to school, or taking care of your family?
8. Continued to drink even when it caused tension with family and friends?
9. Skipped out on or decreased activities you enjoy so you could drink instead?
10. More than once found yourself doing risky things during or after drinking, like driving or having unsafe sex?
11. Have you ever mislead people about how much you drink?
12. Kept drinking even though it made you blackout, feel sad or anxious, or made another health problem worse?
13. Had to drink more than you once did to get the same effects?
14. Felt withdrawal symptoms from alcohol, like insomnia, shakiness, nausea, or a racing heart?
